Grid with Item does not work in four special cases.

Hello,
I use the Grid function to display a solution of Sudoku solver. The blog in Japanese is here.
The following usage of Grid works successfully in many cases, 99%. However, I found four special cases in which it do not work. Would you give me a suggestion how to solve this problem ?
My Mathematica version is 10.0.2.0.

I made the summary concerning Grid Item problem.

From this table, the following temporally rule makes no error.
1. If the color of a number 1 is yellow, the first coloring operation is yellow and the second is green.
2. If the color of a number 1 is green, the first operation is green and the second is yellow.
